He ws trying to pick up a stack of detected bolts (since his x-bow was really his best bet), in what he thought was a safe corner of the dungeon. He tunneled through the wall, into a room he thought to be empty (detect evil was his only detection spell) and before he could even set foot inside he was blasted 5x by nether hounds. Full HP/SP to death instantly.
