Happened during the tainted water quest:
I already cleared the area, but then accidentially walked out of the map, before I could drop the potions. What a mess!
During this frst battle I used up all my healing potions, thus planed a new strategie for the next time: Since greater mimics and magic mashroom patches caused the most problems, I decided to genocide them. Went back to home, grabbed my Agannazar's Antique Acorn (genocide) and my Amulet of Telepathy, to discover them as soon as possible. Everything worked fine so far, genocided greater mimics (seems that *all* mimics where genocided, not quite sure) and made my last and fatal mistake: Simply forgot to swap back to my Amulet of the Magi, which was my only source of free action in that certain eq-combi.
Got paralyzed, then surrounded by spectators which slowly nibbled me to death. Horrible!
