Forgetting to talk to Galadriel after killing the Necromancer, I killed Sauron five times before realising that I hadn't officially got the One Ring Quest yet. That was very, very annoying. Almost as annoying as the Aether Hound on Mount Doom who decided to breath Force at me while I was standing over the Great Fire, knocking me away, and waking everything up. Fortunately I was at +50 speed, so I was able to drop the Ring and get out in the nick of time.
