I was happily whacking lesser balrogs to save the fair princess, when all of a sudden, I was dead. Oops... what was that?? A nycadaemon... must have been summoned by the balroggies. Starting up tome again just so I can see the text, it seems that it breathed nether. I had nether resist, of course, but not enough HP.
