I suppose this death could have been avoided if I had been a bit more careful to check the vault while detecting monsters and had then decided to avoid it completely. As it is, Atlas confused me so I couldn't teleport away, and then started dealing damage faster than I could drink potions of healing. So I guess that's that.

Atlas is normally found at 3800'. Your best bet was staff of teleportation (which can be used when confused). Don't attack Uniques that are 1500+' OoD, because they will kill you. That's what the wand of Teleport Other is for.
