As we've established, you are an unusually lethargic player.
If you can teleport into it, then either it's not a vault (e.g. it might just be a templated room instead), or it was damaged by earthquake or destruction, which remove the "icky tile" modifier that makes the square an invalid teleport target.
Teleportation (of any kind) cannot land you inside of a vault.
The wand of TO isn't in the vault; it's in the far right of the picture.
Alright, new plan. Gonna ?phase from a square room into a nearby hallway which leads to the wand of TO. Hopefully it won't take half my stack of ?phase to make the jump. This is a big shortcut that bypasses Smaug. I'm gonna try to ?phase from the upper left room with the ? in it into the nearby hallway southeast of the room. The wand of TO is just offscreen below the limits of the image.
But first to ?phase out of this room I teleported into ... wow, I have all the luck! Couldn't have gotten a better phase (I ?phased from next to the black ogre in the square room to the left of where @ is).
You can see how close I am to the room with the ? in it, in the upper right of the image, where I will try to ?phase into the hallway.
Well, ?phasing into the hallway was a dumb idea, when 3 charges of my stone to mud does the trick. I don't have enough ?phase doors to and the first 5 tries failed. Odd, there is nothing dangerous in the vault; an in depth Ranger Chieftain is the most dangerous thing. I found a rod of detection on my way to the wand of TO.
We are in business, boys! The vault has a rod of TO. And seething bolts to go with my light xbow of power. And here comes Smaug from below the vault. Ahh, it's nice to catch a break.

At the end of the long hallway is an open oval shaped vault. And standing at its entrance, with the entire hallway in his LoS is Össe, with his 550 nether breath attack. I walked down the hallway unaware of him and then he showed up on my ESP and so I ?teleported out of there (I couldn't TO him because there are also plasma hounds flitting about, and one was in front of him).
How can I lure Össe down the hallway? I don't have any means of digging. I'm afraid that if I walk down the hallway he will just breathe on me for 399 damage and kill me (that is my total HP). Does the damage his breath do dissipate the farther he is from me? Should I walk down the hallway and try to TO him? I could haste self first, up to +17 speed. I did just acquire rNether.
I do have 12 ?charging at home, could blast the first 9 squares with the wands since I won't need them much for this vault. But, yeah, it's fun to go for low turn count wins, but I think you are right here, haha.
As soon as I started digging, Össe came down the hallway! TO time. wish me luck in not dying.
Boy, can't be too careful in this kind of vault. I saw that Kavlax woke up, but I didn't realize he could just walk out of his cubby. And then I didn't pay attention to the brown p I was whacking at, since the last brown p I just killed was an easterling warrior. I should check *every* monster I fight but it gets tedious. So the Berserker quickly had me down to 60s HP and I'm in the middle of the corridor and Xaren is behind Berserker and Kavlax breathes acid from the entrance of the corridor after I !CCW so I had to !heal and ?phase (and hope for the best). Teleport is no longer a safe escape since it'll prolly take me to all the dangerous uniques I TO'd. More later, I have work for 5 hours.
Picked up Belegennon; if I wear it I lose rNether and rChaos but I go from 3.0blows/rnd to 4.0blows/rnd. Seems like it might be worth the trade off at this stage of the game, but not sure. {...later...} A *wing breathes 406 nether and takes me to 37 health. Still, I haven't returned to town to swap back to Himring. I'm CL37 and now on DL91 (if I get them, I read them). At least I have Trickery, +27 speed, rChaos and rDark. Fortunately I was blessed with two or three !Augmentations and !CONs.
Okay, I have +27 speed, and an xbow of extra shots. I fire a bolt, and the Colbran moves one square next to me. Wow, you're that fast? Really? Shouldn't I get like 4 shots on him before he even knows what hit him? I totally don't get that. Then I melee him and take him to half health and he strikes me with electricity and destroys my DEX ring. Amazing.
Movement is more complicated than you are assuming. Your arrow shot took half of a turn. The colbran gets a chance to move after every shot. That he happened to move after your first shot was a roughly 1/4 chance. And it's no surprise he broke your ring. Melee with colbrans (or great storm wyrms, etc) without electricity immunity is not recommended.
But my speed is much faster than his. I thought this was a turn based game. Don't I need to finish my turn before he makes his turn? I thought monsters could only get a turn in between extra shots if they are faster than me.
Another question: does ?Teleport level do anything on DL99 and DL100 if the bosses are alive when playing with forced descent on?

You get a turn when you have at least 100 energy; same for the monsters. The rate at which you accumulate energy depends on your speed. So if you have 100 energy and the Colbran has 99 energy, and you fire a bolt (which, with "2 shots/round" takes 50 energy), you'll be at 50 energy, the Colbran at 99. Guess who gets to 100 first?
Hmm... I guess it makes sense, but usually monsters don't do anything between my extra shots unless they are very fast.
Anyway, I no longer have to worry about Colbrans or anything else that likes to electrify, thanks to Vilya. Amazing, really, given this game has been a massive TO fest and no good vaults (found Arkenstone on the ground, MoD in a normal room), but once again, without my really even realizing it, @ gaining power rapidly, so I've started risking some easier Uniques like Itangast who dropped Vilya.
I seem to get faced with the same dilemma every time I play this game: which missile launcher to use. Since I've pretty much been crash diving the last 40 dungeon levels, I haven't maxed out my stats, so I'm wearing Belegennon for the +4 DEX. Now I found a bow of Lorien with +4 DEX but I currently have 19 slay evil bolts that are good for 500dam/rnd. I don't have rDisenchant, (though I just found a dwarven balance scailmail) but because my light xbow and bolts are so good, I really like the versatility of doing massive ranged damage. However, this Lorien bow gives me +4 DEX, meaning my STR and DEX are both maxed out, giving superb melee damage (I have a *slay evil* mace of disruption at home, waiting for such stats, though this BoC is no slouch). I can probably find arrows, (there are already some mithril arrows on the level I'm currently on) but it's a shame to say goodbye to all my great bolts.
So I ID'd the Lorien bow; pBlindness is one of the better abilities it could have had as it now frees me up to find a good helm. Hammerhand or a good Might would be good. Plus it has high to-dam (+24). Decisions, decisions.
Umm... I _Destruction as Morgoth approached and forgot I had to TO him first. He has not reappeared and I have been all over the level. Shouldn't he be pursuing me?
I've killed all the monsters in the level and explored it thoroughly. No Morgoth.
I have forced descent on and left my recalls at home
If I had known that I would have brought my recalls with me, destructed morgy and mined for consumables, rinse repeat until I'm ready.
I only have 2 *healing* 33 !CCW and 40 !CLW potions, but 2-3 banish, mass banish, ?destruction and 9 teleportation scrolls. I actually forgot to buy ?phase so I have none of those which will make things a bit difficult.
