What about Farmer Maggot? Is he gone now? It really helps to get that one slightly-better item when you're just starting out. I generally do a couple of runs down to Level 1 to get to the point where I can survive the other townspeople and then run around the periphery of the town waiting for Maggot to show up. (I just updated from 3.5.0 so I'm a little behind the times. Getting married, moving to another country, that sort of thing really eats into your Angband time!)
I'm pretty sure Nethack had alignment and thus consequences of some sort for killing townspeople (and/or your little dog). I vaguely remember that if you prayed to your god and you'd been killing innocent townspeople, you might get a smiting instead of a favor.
Somewhere I still have the code for a very silly Nethack mod my ex-roommate and I made, including such character classes as the *ssh*le (scusi) (a Tourist with innate Identify), the Bulldyke (a Valkyrie who started out with all stats maxed), and my favorite, the Guitarist. The guitarist had maxed CHA, a Strat* instead of an Axe, guitar picks instead of missile weapons, and could attempt to charm monsters by playing a solo. Nymphs would giggle when charmed and throw their panties, which were wearable as a +1 helmet. If I recall correctly, guitarists also prayed to Steve Vai. (We were really, really into Steve Vai at the time. Give me a break, it was the 80s.)
The new classes screwed the game balance completely, but it was a lot of fun.
* Personally I prefer a Tele, but Strat just looked cooler.
