Well, it didn't help that when I ran into him, something else had just summoned a pack of Impact Hounds... I've dealt with MMs before, but even at speed +15, I never got a chance for an action so I could get myself out of there.

And I thought it was cool to get "killed by open floor." This is WAY better.

Perhaps by falling through a trap door with minimal HP?

This Flask weighs 1.0 lbs. It is balanced for throwing. When applied to coat an arrow, bolt, sword, or polearm, it hits to blast one target with chaos for 31-30 damage...

Now that's chaotic!

Went something like this, roughly:
7th level Ranger at 50 ft of dungeon:

You have no more scrolls titled "neep pro" of Summon Monster.
The Nether Wraith touches you.
You feel your life force draining away!
You have forgotten the spell of Phase Door. (doh!)
The Nether Wraith touches you.
You die.

Since that episode, I always read unidentified scrolls on a staircase.
