Dead at clvl40, while doing a quest for 20 spider quylthulgs - who summoned Araneas and Elder Araneas almost all the time, and even the ability to duck into mountains didn't save me. The spider-summoning got really out of control and it was either take off the Anchor or mass-geno. Let's just say I didn't realize there were *that many* spiders in the room I was facing. Dead by mass genocide, that's a classic.
