It's hard to know for sure whether you could sensibly have taken a turn in this combat to quaff slow poison. Speaking of this, I'm not certain but you may find it a little easier with a little more Evasion even with the protection-heavy strategy.

I notice you put a couple of points into Song but didn't take any Songs. You might find Song of Elbereth helpful for getting you out of some of the situations where you get ganged up on -- but be warned that it doesn't work on unintelligent monsters, and you can't leave it until the last moment to start singing.
