I met the friend who told me about it in 1987, but I don't know when he told me the story.

I guess I should have added some context. D&D already had a couple of well-known artifacts, the hand of Vecna and the eye of Vecna. The way you use those is to replace the char's hand or eye. So everyone "knew" how the head should be used, and that's why it was a mistake for the head to have both eyes in it.
