[O] Levels are generated with no stairs at all

I just had to quit a promising O character (in 1.10u) because she landed via Word of Recall on a level with no stairs, either up or down; and I unfortunately had no means of teleport level, recall, and whatnot at the time. Anyone know what might cause this, and how I can fix it?

Is it possible the stairs were stuck in a disconnected room or did you have mapping?

Well, that's bizarre - I've never come across that one before. If you still have the savefile, send it to me and I can (a) fix it and (b) try and work out what could have gone wrong.
