    Is this intended?

I know that players cannot teleport or phase door and land inside a vault but apparently it is possible for monsters that you TO to land in a vault. I TOed Fundin Bluecloak, twice, from elsewhere in a level. I finished clearing the level and headed over to the unopened vault. It's a spiral style vault with the open area in the middle and the entrance is buried in granite and Fundin is now inside the vault.

That is, in fact, Fundin's twin brother. I told them to stop switching spots as it confuses people. Dwarves gonna dwarf, ya know?

I suspect this is one of those many things that was basically never intentionally addressed one way or another. So, no, it's (probably) not strictly intended, but that's not to say it's not the desirable behavior either.

Sounds like a feature, not a bug. There's no danger to monsters being teleported into a vault, but possible instadeath for the player. I'm pretty sure that all vaults are small enough that TO from inside the vault won't put the monster somewhere else inside
        the vault.

I had something similar happen recently. I teleported a unique (it might have even been Fundin Bluecloak as well) from one vault and he landed inside another vault (the second one was still sealed so he couldn't have just strolled in).

I have got that with Morgoth. As usual I TO him at one point of endfight, wiped out the summons and healed up full and waited him to come back. And waited and waited and waited....

Then finally I went to find him thinking that maybe there is a limit how far he actually can detect me, and found him trapped behind a permanent wall in a closed (smallish) greater vault.

Uh, this isn't supposed to happen. Radagast just teleported me into a vault, conveniently surrounded by nasty beasts. For the curious, I was about two steps away from him, approaching to melee as missiles weren't doing much compared to his spells.

After posting I notice that this image is illegible. Could someone expain how I can get an HTML screenshot from Angband on Windows so I can post to the screenshots collection here?

I think this qualifies as a bug. I think what you got was tele_to with no room next to Radagast, and game tried to find clear space close by. That check might bypass teleport into vault check.

And it happened again, was inside a vault and a quylthulg teleported me to another place in the vault. I'm getting close to the endgame so I guess I'll be very careful about keeping HP up. The times I've cast phase door to exit a vault it's worked as expected.

I'm not sure if monster teleporting you inside a vault when you are already inside a vault or monster is inside that vault counts as a bug. But you should not end up inside a vault from case where teleport initiator is outside of the vault no matter if it is you or the monster doing that.

It should not be a case of who is doing that, but case of where it is being done. IMO phase door inside a vault should be able to land you inside that same vault.

The monster "teleport player to" spell specifically doesn't consider vaults - so if the nearby square that gets chosen is in a vault, so be it. Mostly this happens when the monster is also in the vault, but clearly not always.

Maybe any hostile teleport should allow landing in a vault

Maybe any hostile teleport should force landing in a vault! And if there's no room you just die. (Or you're forced to become the monster on the square you landed.)

I've had characters die in FA from being teleported into a tree.

I haven't played seriously or looked at any Angband code since the 3.0.x versions, but I thought that phase door was the last resort for getting out of a vault. I certainly haven't logged as many game turns as you but I've never encountered this. Do you remember instances of phase dooring to another part of a vault?

After I clear a large vault I'll cast a few phase doors to see if I can duplicate this.
