Can I only find these at level 99 and level 100, respectively? When and how do they appear? I think I'm almost ready, although I would like to find another one of those powerful rings...

Correct.

When and how do they appear?

They will be on the level when you get there. Sauron you may have to find; Morgoth should find you...
