    Options question

One of the settings in the interface options is "forgo bonus attacks on non-alert enemies" but as far as I can tell it's never very clearly explained exactly what a "bonus attack" is.

I assume this doesn't mean the to-hit bonus you get for attacking unwary monsters, so does this mean the extra attack you get from things like follow through or whirlwind attack?

I want to say "attacks of opportunity", I'm sure someone who actually knows will correct me.
One for the Dark Lord on his dark throne
In the Land of Mordor where the Shadows lie.

Yes. Any attack you get on an enemy which is not a direct result of moving into them should count - this would include also Zone of Control, or Polearm Mastery attacks. I'd need to look at the code and refresh my memory to come up with a complete list, but in general this is most likely to be useful if you have some skill that grants extra attacks at targets you haven't specified.

        What Quirk said.

It's there for stealth characters that don't want to wake things up by accidentally hitting them with flanking or opportunist.
