For me, this was an once in a lifetime speed ring drop:

d) a Ring of Speed <+20> {!d}
Dropped by a Sorcerer at 2650 feet (level 53)

+20 speed.

Just a regular mage pit in DL 53.
I cannot imagine the odds.

Good question. I did play Moria a lot back in the days and never saw one generated, even when experimenting on a wizard mode.
Was it even possible? I'm pretty sure found RoS +1 just once (without wiz) and screwed it up by not having free action.
