Death to extreme stupidity - I was hunting Quylthulgs and I found a room with a lot of powerful monsters. So, instead of thinking "how did they get here", and using the phial to illuminate it and locate the Quylthulg, I started fighting them off - and didn't even realize more were joining. By the time I realized, I had 0 hp, I was cut, and surrounded by monsters. So, instead of drinking the potion of CCW which might have given me a chance, I teleported away and died from the cut.

So much for playing while watching TV... Doesn't really help concentration.
