Old Z, vanilla town, small levels, disconnected stairs. Did all 49 random quests and killed Oberon, then died very soon after starting to scum for more healing potions.

The level that killed me was open and had several groups of nasty monsters. I teleported back and forth between them, then at 200 HP decided to try genociding U since I would probably be okay if it didn't kill me. It did.

A scroll of teleport level or *destruction* would have saved me of course, but I was out of them and they don't normally generate in stores in this version.
