It's also a bad idea to stab monsters in stealth mode -- you get +5 to melee from the stealth bonus, but they generally get at least one hit on you in exchange, and if they're fast they'll get two.
You get a stealth bonus for:
a) having walls around you (the more the better)
b) resting (this one is huge, +7)
so you just sort of have to learn how to screw around with the stealth mechanics so you can move at normal speed and only use stealth mode as a desperation measure, or to sneak past a room full of bad guys.
Sneaking around in clear sight is also a bad idea.
If you can deal with breaking your stealth/melee/evasion pact, putting a few points into Song and taking Song of Silence can help a lot with noise, even at low scores.
You have high str and low perception, which leads me to believe you're probably trying to smash down doors also. That's not a good idea :) In general, low percept goes badly with high stealth. Once you start setting off traps, things will find you real quick.
If this was me, this would be the stage in the game where I go for loremaster. Listen is also super helpful for stealth builds, but you seem to dislike that idea :) Dodging + sprinting works well with vanishing (even if you don't have the Vanish skill), since you get out of LOS faster -- that might be a good next option.

I was thinking of the dodge-sprint vanish combo too, after getting into some hairy fights.
Perception would be great, but I would want focused attack first, then keen senses/listen. I just don't mind the ID game anymore. I love listen, and have moved from the "i-need-light-and-more-light-camp" to, "I can just hear them and talk halvesies". I might break the conduct, but I'd like to see how far I can take this.

But basically walking around Slow is a bad idea just for safety reasons :) It's not so bad once you get sprinting, since you can stay in stealth mode while running in one direction, and the running bonus will cancel out the slowness. Not sure what happens if you assassinate someone in that state, that would be a neat way to get the +5 bonus without risking a double-hit in retaliation.

What a shame... I was finally starting to enjoy stealth in just the last few hundred turns. I learned some good things, and thanks for all the advice/comments.
About flanking... no, I hadn't considered it. I'm leaning towards a nah, but I can see how flanking would be good.

I don't understand debo's comment on the sword of Galadriel. You use it only in the off-hand when two-weapon fighting? Personally I like the hunting blades too. Amrod in particular is really cool when you are playing a stealthy character. If you use perception (focus) as well doubly so.
Galadriel would probably have done even more damage to the red d. Not to talk of Amrod.

Galadriel is great for a stabby build, no doubt. But the way this guy was built, evasion was quite low anyways, so riposte is sort of not helpful. Once you take away the neato evasion bonus (which is good) and riposte, you just have an ideally weighted +0, 1d8 shortsword. You can forge that without too much trouble yourself.
Amrod and Dagmor are both way better for assassinating, IMO. Amrod give you effective +3 or 4 to melee when assassinating (can't remember what the base melee bonus on it is, and +2 stealth), and Dagmor is just badass b/c of +2, 2.0lb, and the point of grace and the light and all the other awesome. The goal with my stabbers is really to kill or completely cripple the enemy on the first hit. Galadriel is great if you need to follow-up with something :)
I tend to build stabbers with relatively low evasion too, which is fragile but immensely satisfying :)

I was actually trying to just stealthily nab some loot in the back of the vault. I probably should've turned on sneak mode since I knew I would die with or without slowness anyway. Oh well!
I would normally use perception, but I arbitrarily said I would try to go with only melee/evasion/stealth, so I stuck with it.

Also, in recent stabber games I tried diving past CatLand entirely (dive from 650' to 850'), grabbing some XP and good stuff from the deep depths, and then going back up for the XP -- with mixed results. And by "mixed", I mean "dead".
This was a brave challenge, doing things without perception. I think you might be surprised at how much easier it gets if your perception goes up a bit.
I also wonder how rushing to listen would affect the early game...
