Do I get aim penalty if I shoot a monster standing next to me?

Not in V. Don't know about other variants.

You do get an aim penalty when the monster is further away though. It increases with distance. I can't remember what exactly the penalty is though, but I seem to recall it being reasonably substantial when trying to hit monsters ~10 tiles away.
