Just fought off a horde of mutually reinforcing demons, a truly epic battle during which I leveled from 37 to 39. Had just finished off the last one and was headed back to pick through the scads of juicy drops. Right at the start, ID'd a helm: Holhenneth. Sweet! Went to pick it up. Somehow cast Teleport Level instead and dropped through the floor.

Pounded head on desk for a while.

Re-inscribed Mordenkeinen's Escapes with "!*".

I bet you have Mordencainen's in slot f, and instead of mcf you did mfc. Or I'm mistaken.

Use numbers and inscribe books with @m1, @m2 etc. That way you don't do any mistakes. Also keymaps are very useful tool to do things you do a lot.

These will help, but they won't guarantee that you won't accidentally cast Teleport Level instead of e.g. Teleport Other, or Portal instead of Resist Heat and Cold, etc.
