Sil-Q : guaranteed forge annoyance

Guaranteed forges have a chance of spawning inside quartz vaults. This is annoying when you are given no tools (mattock, horn of blasting, earthquakes stave) to circumvent this. This can arbitrarily make smith runs much, much harder (guaranteed forges are their only saving grace already).

Please look into this, and hopefully remove it.
