    Taking on the Mouth of Sauron with 13 speed and at clevel 36 seems a tad... optimistic. In other words: I don't think were actually that "close" .

      I wasn't trying to take him on. I was trying to teleport him away to pick my way through a large vault. But... There was a different monster I did not see in between him and me that I teleported away instead.

        Read "be anywhere near him on the level" for "take him on". The two seem to be largely equivalent in Angband .

          He had two turns before you tried to teleport him away. If he had used both of those to either manastorm or darkness storm you you would have been dead anyway.

          Angband is a game where small probabilities stack up pretty fast because there are so many times you hit that small probability. Don't take even small chances because eventually it will backfire. 99% success means you probably will hit that 1% in next 100 tries, and 100 is not much in game like angband (also because of the nature of Random Number God you probably hit it twice in a row at the worst possible time, and then go next 1000 non-deadly situations without failure).

            I would say rather "don't unintentionally take minor risks." If, say, there's an item you suspect you really want, like an unidentified ring or a spellbook, and the only way you have to get to it is to allow some nasty monster to spend a turn with you visible and hope that the monster doesn't use their most powerful attack -- it might be worth doing! On the other hand, if you just generally play sloppily because you don't realize that you're at risk of instant death, then eventually that's going to catch up with you.

            In other words, it's fine to take calculated risks.
