Charlston IV got lucky and got Theoden's axe at a really low level. She began diving and was around 1400' or so and bashing away at an orc pit. Slaying baddies left and right, got down to about 80HP, then a slicer death beetle or something was mixed in and I wasn't paying attention.

I've lost a surprising amount of characters to beetles. Usually, as you say, when fighting something else entirely and not paying attention.

... honestly, you'd want to be careful around anything with "death" in its name. Except, of course, for the "trumpet of death" (see QI).
