Checkerboard-one has several "8" positions which are monster 40 levels OoD and guarantee excellent object 20 lvl OoD.
Ok, it was this one, tricked me nicely. But 40 levels is huge, it still doesn't explain Ungoliath, 48 levels OOD. If I read correct vault.txt, monsters on "8" are up to 40 levels OOD or items 20 OOD.
Just last night I happened upon a huge vault at dlvl 10. It was too far away in granite for me to get to with my one Wand of Stone to Mud and I was only a Clvl9 Ranger, so no spell.
I was so dying to know what was in it that I went to debug mode. A Great Wyrm of Law (demonologist, wraiths and 9-headed hydra aside the rest wasn't too scary) was probably the most OOD thing there. Picked up about 20 *egos* but no artifacts.

Not or, and. "up to" in the description is a bit misleading, it is "created like current level + 40 dlvls would be floor depth" for that grid. Monsters can be OoD even further if they pass the normal OoD checks. Description also lacks the "excellent" in item. 8 has something that is guaranteed to be at least ego-item, speed ring, few of the deeper amulets, spellbooks or dragon armors. Artifacts are very common in those.
So Ungoliant was only 8 levels OoD, which is a lot but not unheard of. I have seen AMHD asleep at ~1600' in a room that definitely was not a vault. AMHD is native at 2150' so 11 levels OoD for it. No full basic four or poison resist and ~200HP. Needless to say I avoided any close encounters with it.
